Assumption of Risk:

I, the minor's parent or legal guardian, understand the nature of the activities my child will be involved in at Chicago Contemporary Dance Theatre, and the minor's experience and capabilities, and believe the minor to be qualified, in good health, and in proper physical condition to participate in such activities.


Release of Liability:

I hereby release, discharge, covenant not to sue, and agree to indemnify and save and hold harmless each of the releasee's from all liability claims, demands, losses, or damages on the minor's account caused or alleged to be caused, in whole or in part by the negligence of the "releasees" or otherwise, including negligent rescue operations. I further agree that if, despite this release, I, the minor, or anyone on the minor's behalf makes a claim against any of the releasees named above, I will indemnify, save, and hold harmless each of the releasees from any litigation expenses, attorney fees, loss liability, damage, or any cost that may occur as the result of any such claim.


Medical Emergencies:

In any event of an accident or emergency I hereby authorize my child to be transported to a hospital for medical treatment and I hold Chicago Contemporary Dance Theatre and it's representatives harmless in the execution of such. Additionally, I hereby agree to individually provide for all medical expenses which may be incurred by me or my child(ren) as a result of any injury sustained while participating at or for Chicago Contemporary Dance Theatre.

Updates, Announcements and more…

All studio updates will be via-email. All information is subject to change without notice.  Please update your email address with the front desk immediately, so that you will remain up to date  


Registration Registering with Chi-CDT is a full season commitment.  Prior to starting classes, each student will be required to pay a non-refundable $40 registration per dance term.



Tuition  Fees are figured based on the number of classes taken by each student and are due quarterly or annually. Tuition that is received after the 7th day shall receive to a $10 late fee per week until paid in full.  A dancer will be excluded from class after 30 days of non-payment.  Tuition is due regardless of the number of classes attended; no refunds will be given due to dropping class nor for classes missed because of illness, vacation, or cancellations caused by inclement weather. Tuition can be paid by cash, check, or charge by credit card.


Referral Program The Referral Program gives our current students a chance to earn $25 towards their tuition.  If you tell a friend or family member about Chi-CDT and they enroll in a dance, you get $25 off your tuition after they have paid at least one full quarterly payment. There’s NO LIMIT! So, start telling everyone you know about our new classes and remind them to mention your name when they enroll!

There are additional fees for the Dance Concert, and other events held by Chi-CDT. 


Returned Check Fee There will be a $50 fee assessed for all returned checks.


Class Start and End Times Parents are responsible for ensuring that their child arrive to class on time and must pick up as soon as their last class has ended. Chi-CDT’s staff is not responsible for children left unattended before or after dance class.  All students must be picked up no later than 10 minutes after their last class.  In the event you are running late please call the studio and inform the front desk that you are running late and your expected time of arrival.  Failure to do so will result in a fine of $5 due next class period.  After that there will be a fee of $10 every 5 minutes that your child is not picked up.


Attendance Please sign in & out.  Regular attendance is encouraged to ensure optimal growth of both individual dancers and the class as a whole.  Excessive absences will result in minimal parts in the dance concert.


Classroom Behavior Dancers are expected to respect their dance teacher, classmates and studio at all times. Chi-CDT staff reserves the right to excuse any student from class if their behavior is inappropriate and disruptive.  If this occurs, dancers will be removed from class and parent will be notified immediately.


Dress Code All Dancers are expected to be properly dressed and ready for each class they attend.  Students that are improperly dressed will be giving the appropriate attire and items will be charged to their account.


students will be given 2 weeks in order to purchase the proper items needed for classes. 

If you need to order any items, you may do as at the front desk.


Leotards must be worn in all ballet classes.  No shorts nor pants allowed

Dance Shoes Dancers must wear the proper dance shoes for each dance class. Dance shoes should not be worn outside the dance studio, since this can cause unnecessary wear and tear to your child’s dance shoes. Dance shoes should be marked with the dancer’s name and carried to and from the dance studio in a dedicated dance bag. NO OUTSIDE SHOES


Washroom Break Please use the restroom before coming to class! Bathroom breaks are provided on an as needed basis. There are bathrooms on every floor of the building. 


Food  No food allowed at all.  Water will be available at the desk for $1.


Waiting Area The waiting area is for those who are waiting for someone in class and or dancers waiting for class. If your child has more than a 60min wait in between classes a parent or guardian must be with them during that time.  Please bring as few items as possible to the studio and all dancers items should be placed in a locker AND LABLED WITH YOUR CHILD’S NAME.  There should be no items on chairs.  Noise should be kept to a minimum.


Changing Area/Lockers There are a limited number of lockers available for dancers.  All items should be neatly placed in locker preferably in a labeled bag.  ABSOULUTLY NO FOOD IN THE LOCKERS!!!!!!!! IF FOOD IS FOUND IT WILL BE THROWN AWAY!    

All Unclaimed Items Chi-CDT becomes the property owner of all items left at the studio over 7 days.  This includes but not is limited to uniforms, costumes, and any other items left at the studio.  Chi-CDT is not responsible for any items left or stolen at the studio.  The lost and found will be disposed of weekly.


Winter Weather-Related Closings   In the case of inclement winter weather, Chi-CDT will cancel class if the majority of the major school districts are not holding classes and activities. If bad weather arrives during the course of the day and parents are unsure about the status of classes for the evening, they can check the studio voicemail, which will always be updated by 3:30 p.m. If bad weather begins to arrive through the course of the evening, parents are responsible for calling the studio to determine whether classes are cancelled before coming to class. Chi-CDT will not typically make up classes cancelled due to bad weather unless the same class misses two or more sessions, in which case Chi-CDT staff will attempt to reschedule the class.


Holiday Breaks The studio will be closed during the following holiday breaks: Thanksgiving, Christmas, and Spring Break. Exact break dates will be announced at the beginning of each dance year. Holiday breaks for the current year can be found online at


Where to Park- Do Not Park in the gated parking lot.  That parking lot is for renters of the Bridgeport Art Center only. You may park on Racine, on 35th street, or in the parking lot surrounding the gated parking lot.  The main entrance to the studio is on Racine.  Do Not double park even if you are dropping off your child.  Chi-CDT will not be responsible for your car being towed.


Policy and Procedure Changes Chicago Contemporary Dance Theatre reserves the right to make changes to the policies and procedures at any time.  Notice of changes will be given in an email and posted at the studio.
